Edinburgh, 1850. Despite being at the forefront of modern medicine, hordes of patients are dying all across the city, with doctors finding their remedies powerless. Back in the townhouse of Dr James Simpson, Sarah Fisher has set her sights on learning to practise medicine. Almost everyone seems intent on dissuading her from this ambition, but when word reaches her that a woman has recently obtained a medical degree despite her gender, Sarah decides to seek her out.

Young women are being discovered dead across the Old Town, all having suffered similarly gruesome ends. In the New Town, medical student Will Raven is about to start his apprenticeship with the brilliant and renowned Dr Simpson. Simpson's patients range from the richest to the poorest of this divided city. His house is like no other, full of visiting luminaries and daring experiments in the new medical frontier of anaesthesia.
