Spanien, 1944: Die junge Ofelia reist mit ihrer schwangeren Mutter zu ihrem unbarmherzigen Stiefvater Capitán Vidal in eine verlassene Mühle, die seinen Truppen als Stützpunkt zur Bekämpfung der verbliebenen Rebellen dient. Auf der Flucht vor dieser grausamen Realität findet Ofelia ein verwittertes Labyrinth, das ihr ein Tor in eine fantastische Welt öffnet. Dort trifft sie auf einen Faun, der in ihr die verschollene Prinzessin des unterirdischen Königreichs erkennt und sie vor drei gefährliche Aufgaben stellt.
