In the early 1990’s, Hellboy, Liz Sherman, and Abe Sapien are drawn into a bizarre case in New York, linked both to Hellboy’s 1980 adventure with Anastasia in Brazil and to The Lobster’s encounter with the Proteus Club in 1933. A strange community of bizarre humans has been living in secret beneath Manhattan for more than sixty years. Now, though, someone is stalking them, murdering them one by one—someone who claims to be the ancient goddess Sem-Karul. With aid from the now elderly Cynthia Tynan, Hellboy and the BPRD must face the Goddess of Manhattan, and the monsters who serve her.

Jane Watkins, reporter for the Seattle Gazette newspaper, goes to meet a source with information about a serial killer known as the Slasher - only to find herself facing not an informant but the killer. She nearly meets her death but is miraculously saved by a glowing figure. He calls himself the Advocate, and Jane's encounter with them both is about to change her life and theirs forever. It seems the Advocate - whose real name is John Tomar - is one of several people who have developed superhuman powers through a substance known as the Genesis Factor.

The general population refers to them as "Odds", people who suffer from AOHD (Adult Onset Hyper-Development Disorder). Such people wake up one day to find that they're suddenly super-human. They can do things like leap tall buildings. Lift bulldozers over their heads. Read minds. Throw fireballs. Melt steel with a thought. Fly at supersonic speeds, and so on. And what happens? Uncle Sam makes them sign up with the Feds and punch a clock. Or else.
