Reaching the famous “zero accidents” objective by making employees more aware of their responsibilities in terms of health and safety, as well as making them behave safely at work is a little like the Holy Grail, the treasure coveted by practically all organizations as the solution to virtually all industrial accident prevention problems.

In the 1930s, William Herbert Heinrich successfully drew attention to workplace accident prevention. After his publications, we saw two ratios emerge as popular beliefs in health and safety, which radically changed the prevention world. The first ratio was 300-29-1: 300 small events would lead to 29 injuries and finally a major event. After this first ratio, a second emerged, namely the 88-10-2 ratio, which maintains that 88% of events are linked to human behavior, 10% to environmental conditions and 2% to chance.

For those of you who don’t like the idea of applying military principles to health and safety management by imposing a merciless, prescriptive hierarchical culture, don’t worry. That is really not what this book is about. For thousands of years, the military world has by far been the most advanced in terms of strategy, and we have much to learn from it when it comes to preventing work accidents. Strategy must include the knowledge and understanding of the roles and responsibilities of all players within organizations, a recurring problem in health and safety systems.
