In 1900, the U.S. sent three doctors to Cuba to discover how yellow fever was spread. There, they launched one of history's most controversial human studies. In Medical Bondage, Cooper Owens examines a wide range of scientific literature and less formal communications in which gynecologists created and disseminated medical fictions about their patients, such as their belief that black enslaved women could withstand pain better than white "ladies". Even as they were advancing medicine, these doctors were legitimizing, for decades to come, groundless theories related to whiteness and blackness, men and women, and the inferiority of other races or nationalities.

In 1932, the US Public Health Service recruited 623 African American men from Macon County, Alabama, for a study of "the effects of untreated syphilis in the Negro male". For the next 40 years - even after the development of penicillin, the cure for syphilis - these men were denied medical care for this potentially fatal disease.
