Have you ever taken your Parkinson’s medication, waited for it to kick in, and wondered, “Why isn't this working anymore?”
You're still stiff. Your feet aren't cooperating. Your brain feels foggy, and suddenly you're wondering whether your Parkinson’s has gotten worse.
But sometimes the medication hasn't stopped working. Something else has changed.
In this episode of Living with Parkinson’s: The Good, the Bad, and the Reality, I share five reasons your medication may suddenly seem less effective:
• Delayed ON time: Your medication may still be working, just taking longer to kick in.
• Food and protein timing: Why meals, particularly protein-rich foods, can affect levodopa response for some people.
• Constipation and slow digestion: How your digestive system can interfere with medication absorption and make doses unpredictable.
• Sleep, stress, dehydration, or illness: Why a difficult day doesn't always mean your medication has failed.
• Symptoms that don't respond well to levodopa: Why increasing medication isn't necessarily the answer to fatigue, balance problems, brain fog, or other symptoms.
And here's the question I now ask before assuming my medication has stopped working:
“What else changed?”
Did I sleep poorly? Eat differently? Take my medication later? Am I constipated, dehydrated, or under more stress?
Sometimes asking that question turns panic into useful information.
You'll also hear Carmen’s Care Partner Corner, where Carmen talks about supporting someone through difficult medication days without constantly asking:
“Are you ON yet?”
Her reminder is simple:
“You don't have to solve the whole disease. Sometimes you just help make the next hour easier.”
Because one bad medication day doesn't automatically mean your Parkinson’s has progressed.
Sometimes the pill didn't fail. Parkinson’s just changed the rules.

“With Parkinson’s, medication is like coffee. It’s not about how strong it is. It’s about when you take it.”
⚠️ Important Note
This is my personal Parkinson’s journey. What works for me may not work for everyone. Parkinson’s is different for each person, and medication plans should always be discussed with your doctor or healthcare provider.
